Dear Patient thank you for using AppointMentor online booking facility. When you book an appointment it is booked as a provisional appointment until a staff member can contact you for payment and confirmation.

Cancellation Policy

I consent to North Lane Dental Practice’s Cancellation Policy as stated below;

For Cancelling and rescheduling appointments a minimum period of 48hrs notice is required,

I am aware that if my appointment is cancelled without 48hrs notice or failure to

attend without notice will result in the following fees, 30-minute appointment = £50, 60-minute appointment = £100, 90-minute appointment = £150, Half a day (3 hours plus) = £250 and Whole day (5/6 hours plus) = £500. For appointments cancelled with the notice of more than 48hrs will be refunded the total amount paid for the appointment booked. Further appointments shall be offered at the practice.

North Lane Dental Practice’s Privacy Policy

Our online booking system is managed through a trusted 3rd party called Welltime Ltd who acts as a data processor that provides the AppointMentor online booking service, in compliance with the UK laws and GDPR.

For example, your name, date of birth, gender, address, telephone number, email address and occupation etc. Information about your dental and general health, including clinical records made by dentists and other dental professionals involved with your care and treatment. X-rays, clinical photographs, digital scans of your mouth and teeth, and study models, medical and dental histories, treatment plans and consent, notes of conversations with you about your care, for example details of any queries, complaints and any related communications etc.

We may collect or pass on your information to and from other health professionals, such as your doctor, specialist practice or other medical institutions.

You may give us your personal information through filling in patient forms, providing your details through our social media accounts, Instagram and Facebook, or by providing your details on our website. You may also correspond with us by phone, e-mail, chat or otherwise.

We have CCTV in practice for the purposes of security and the safety of our staff and patients.

By visiting our website, and accessing or using communications such as social media chat, or our chat bot, you are consenting to the collection, use, storage, processing and disclosure of its information in accordance with our Privacy Policy.

We may need to share your information with other medical and health professionals as well as dental specialists, dental labs, private dental health schemes or any health professionals who care for you. We keep your dental records for 10 years after the date of your last visit and we will only keep information, such as records through chat functions, for as long as necessary to fulfil the purposes we collected.

You have the right to access your information that we hold in practice and on file for you. You can make a request to access the information we do hold, by contacting your practice or by e-mailing enquiries@northlanedental.com

You have a right to correct any information that you believe is inaccurate or incomplete. Please contact your practice to request a change in information.
