Dear Patient thank you for using AppointMentor online booking facility. When you book an appointment it is booked as a provisional appointment until a staff member can contact you for payment and confirmation.
I consent to North Lane Dental Practice’s Cancellation Policy as stated below;
For Cancelling and rescheduling appointments a minimum period of 48hrs notice is required,
I am aware that if my appointment is cancelled without 48hrs notice or failure to
attend without notice will result in the following fees, 30-minute appointment = £50, 60-minute appointment = £100, 90-minute appointment = £150, Half a day (3 hours plus) = £250 and Whole day (5/6 hours plus) = £500. For appointments cancelled with the notice of more than 48hrs will be refunded the total amount paid for the appointment booked. Further appointments shall be offered at the practice.

We may collect or pass on your information to and from other health professionals, such as your doctor, specialist practice or other medical institutions.
You may give us your personal information through filling in patient forms, providing your details through our social media accounts, Instagram and Facebook, or by providing your details on our website. You may also correspond with us by phone, e-mail, chat or otherwise.
We have CCTV in practice for the purposes of security and the safety of our staff and patients.
